1. Agreement to Terms
Welcome to North Star Phoenix. These Terms & Conditions ("Terms") govern your use of our community volunteer coordination services, mobile applications, automated telephone hotline, and SMS messaging services. By enrolling as a rider ("Friend"), volunteering, calling our dispatch line, or using our text messaging services, you agree to be bound by these Terms.
2. Community Transportation Program Description
North Star Phoenix coordinates volunteer-assisted transportation and community help for seniors and individuals in need across our local service areas. Registered participants are issued a unique 4-digit Friend ID. Community members can use this Friend ID to request pickups by dialing our automated telephone hotline via touch-tone phone or by sending a text message with their Friend ID when ready to return from medical or personal appointments.
3. SMS Messaging Terms & Conditions
North Star Phoenix provides transactional text messaging to keep seniors and volunteers informed regarding ride confirmations, dispatch updates, and return pickups.
Program Name: North Star Phoenix Transportation & Pickup Alerts
Program Description: Automated notifications confirming that a senior rider's call-in or text-in pickup request has been received, that a volunteer driver has been dispatched, and real-time arrival estimates.
Message & Data Rates: Message and data rates may apply for any messages sent to you from us and to us from you.
Message Frequency: Message frequency varies based on the frequency of your scheduled rides and pickup requests (typically 2 to 4 messages per scheduled round trip).
Opt-Out Instructions: You can cancel the SMS service at any time. Simply text STOP to our number. After you send the SMS message STOP to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, reply START or sign up through our dispatch office.
Customer Support: If you are experiencing issues with the messaging program, you can reply with the keyword HELP for more assistance, call our office, or email us at support@northstarphoenix.com.
Carrier Liability Disclaimer: Carriers are not liable for delayed or undelivered messages.
Privacy Policy: For details on how we handle your mobile number and personal data, please review our Privacy Policy. As noted in our Privacy Policy, we do not sell or share SMS opt-in data or personal information with third parties for marketing purposes.
4. Telephone & Interactive Voice Response (IVR) Service
Seniors may call our dedicated telephone hotline and punch in their 4-digit Friend ID using their phone's keypad. By calling and entering your Friend ID, you authorize North Star Phoenix to access your pending ride details in our dispatch system and notify your assigned volunteer driver that you are ready for pickup.
5. Volunteer Services Disclaimer
North Star Phoenix operates as a community volunteer network. Rides and services are provided by community volunteers in good faith. While we make every reasonable effort to ensure timely dispatch and reliable volunteer matching, North Star Phoenix does not guarantee immediate availability for all requests and is not an emergency medical transport service. For medical emergencies, always dial 911 immediately.
6. User Responsibilities
- Provide accurate contact details, destination addresses, and necessary mobility requirements (such as walker or wheelchair accommodations).
- Keep your 4-digit Friend ID handy when attending appointments to ensure quick pickup coordination.
- Treat volunteer drivers, dispatchers, and community members with respect and courtesy.
7. Termination of Service
We reserve the right to suspend or discontinue access to our volunteer programs or automated communication channels for anyone who engages in abusive behavior, provides fraudulent information, or repeatedly violates these Terms.
8. Changes to These Terms
We may update these Terms from time to time. Any changes will be posted on this page with an updated Effective Date. Your continued use of North Star Phoenix services after updates constitutes acceptance of the modified Terms.
